Transitioning to Adult Care

Health care transition is a gradual process of planning, transferring care, and becoming established in adult-centered care. The timing and team will differ from person to person, so begin early enough to reduce the chance of a gap in care.

Begin the conversation

  • Ask when pediatric care ends and when transition planning begins.
  • Identify which current providers can continue seeing you as an adult.
  • Ask whether the pediatric team recommends an adult NF clinic or other adult providers.
  • Check whether insurance, referrals, authorizations, or consent rules will change.

Build the handoff

  • Request a concise medical summary and current care or monitoring plan.
  • Gather important imaging, pathology, genetic testing, surgery, and treatment records.
  • Ask the pediatric and adult teams to communicate directly when possible.
  • Confirm who remains responsible for prescriptions, results, and pending referrals until adult care begins.

Set up adult care

  • Schedule the first adult appointment before pediatric care ends when possible.
  • Confirm that the new provider received and reviewed the transfer records.
  • Ask who will coordinate NF-related care and how primary and specialty care will work together.
  • If no adult NF clinic is nearby, ask about shared care with local providers and a more distant NF center.

Confirm the transition

  • Review who handles each part of care and whom to contact about a new concern.
  • Schedule ordered imaging, referrals, and follow-up visits.
  • Keep a copy of the first adult visit summary and update your personal health record.
  • Confirm that no pending result, refill, order, or referral was lost during the transfer.
